The following quiz will assess both the level of support and the stage of change that will apply to the quiz taker.

Question 1: For Exercise...
You know someone that you can ask and you are confident that they will support you
You know someone who will support you in your efforts to exercise consistently
You are not certain that you will have any support in your efforts to start and stay with an exercise program
You know someone who is already exercising and will exercise with you

Question 2: For diet/nutrition changes...
You know someone who will support you in your efforts to eat better
You are not certain that you will have any support in your efforts to start and stay with a healthy eating program
You know someone who is currently eating well and will assist you in your program
You know someone that you can ask and are confident that they will support you

Question 3: For stress management...
You know someone who is already managing their stress well and will assist you
You know someone that you can ask and are confident that they will support you
You know someone who will support you in your efforts to manage your stress
You are not certain that you will have any support in your efforts to start and stay with a stress management program

Question 4: For sleep management...
You know someone that you can ask and are confident that they will support you
You are not certain that you will have any support in your efforts to start managing your sleep better
You know someone who is getting enough sleep and will assist you in your efforts
You know someone who will support you in your efforts to manage your sleep

Question 5: Exercise (Stage of Change)
You are currently in an exercise routine but started less than 6 months ago
You are not thinking about starting to exercise and don't anticipate that you will in the next 6 months
You have been exercising for the past 6 months or longer
You will be ready to think about starting to exercise in the next 6 months
You are currently not exercising but anticipate starting in the very near future or within 6 months

Question 6: Dietary Changes (Stage of Change)
You will be ready to think about eating well in the next 6 months
You are not thinking about starting to eat well and don't anticipate that you will in the next 6 months
You are currently eating well but started less than 6 months ago
You are currently not eating well but anticipate starting in the very near future or within 6 months
You have been eating well for the past 6 months or longer

Question 7: Stress Management (Stage of Change)
You have been managing stress well for the past 6 months or longer
You are currently managing stress well but started less than 6 months ago
You will be ready to think about managing stress well in the next 6 months
You are not thinking about starting to manage stress well and don't anticipate that you will in then next 6 months
You are currently not managing stress well but anticipate starting in the very near future or within 6 months

Question 8: Sleep Management (Stage of Change)
You will be ready to think about getting enough sleep in the next 6 months
You are currently not getting enough sleep but anticipate starting in the very near future or within 6 months
You are not thinking about starting to get enough sleep and don't anticipate that you will in the next 6 months
You are currently getting enough sleep but started less than 6 months ago
You have been getting enough sleep for the past 6 months or longer

Question 9: Time Management (Stage of Change)
You are currently not managing your time properly but anticipate starting in the very near future or within 6 months
You are not thinking about time management and don't anticipate that you will in the next 6 months
You will be ready to think about time management in the next 6 months
You have been managing your time adequately for the past 6 months or longer
You are currently managing your time adequately but started less than 6 months ago

Question 10: Family/Friends (Stage of Change)
You have an excellent relationship with family and friends, and have had such for the past 6 months or longer
You are not thinking about forming an excellent relationship with family and friends and don't anticipate that you will in the next 6 months
You currently don't have an excellent relationship with family and friends but anticipate starting in the very near future or within 6 months
You currently have an excellent relationship with family and friends but started less than 6 months ago
You will be ready to think about forming an excellent relationship with family and friends in the next 6 months
