The following quiz will be used to assess your current eating habits. It will primarily be used as a way to gauge your food intake on a day-to-day basis.

    How many servings of fruits do you eat on a daily basis? Keep in mind that one serving is usually equal to about one cup of chopped or sliced fruits, or one medium sized fruit.

    How many servings of vegetables do you eat? Keep in mind that one serving is usually equal to about one cup of chopped or sliced vegetables.

    How many servings of milk, cheese, yogurt or other dairy products do you typically consume? One serving of cheese is about the size of four dice. One serving of milk or yogurt is equal to one cup. If you do not eat dairy, how many calcium fortified foods do you typically consume?

    How many servings of bread, cereal, pasta or other grains do you eat? One serving is equal to one slice of bread or one cup of cereal or pasta.
